Intake Form 2024-25
We require an intake form to be completed for all of our ministry friends BEFORE they can participate in any of our activities. The intake form provides our staff and serving team critical information in order to provide the safest environment for our ministry friends. We ask that a form be completed once a year or anytime throughout the year when something medically has changed for our friends. If you are unsure if you have completed one, please email included@fmcm.org and we will let you know.

Our Included Moms Bible Study is a safe place for moms and caregivers to share life and learn about Jesus. We hope you can join us for this special time together.

The IDD Council’s Making Connections Disability Resource Fair brings together the disability resources in our community in one, easily accessible location. Families and caregivers can collect information and learn about the programs and services each provides.

The Village presents “All the Feels: Navigating Your Child’s Emotions,” a four-week series designed to help parents understand and support their children’s emotional development. Each week we’ll explore key strategies for identifying, validating, and guiding kids through their feelings while equipping parents with practical tools to foster emotional resilience and open communication.
