Welcome to the Included Special Needs Ministry, where we have a place for everyone in your family. We are able to provide the very best for all ages and all abilities. First Methodist Mansfield offers a safe environment to learn about Jesus based on the individual needs of our participants. We believe in having fun while we fellowship, build friendships, serve, and worship. Our Included staff would love to connect with you and see how we can best serve you and your family.
Annual Intake Form
We require an intake form to be completed for all of our ministry friends BEFORE they can participate in any of our activities. The intake form provides our staff and serving team critical information in order to provide the safest environment for our ministry friends. We ask that a form be completed once a year or anytime throughout the year when something medically has changed for our friends. If you are unsure if you have completed one, please email included@fmcm.org and we will let you know.
