We require an Intake Form for each individual to participate in any of First Methodist’s Included activities. We ask that the Intake Form be updated yearly unless there is a change in your participant’s needs, then we ask that you update the form as needed throughout the year.
This form is essential for us to review the needs of each individual and provide specialized care based on those needs. We also use the Intake Form’s contact information in case of an emergency.
